THROUGH OUR APPROVED SERVICE PROVIDER.
PLEASE READ ALL OF THE NOTES BEFORE ATTEMPTING TO COMPLETE THIS FORM.
1. Airtime is provided subject to status and to persons over
18 who
are UK residents.
2. This registration FORM MUST BE COMPLETED IN ADVANCE to
enable us
to connect your telephone. This form is for registering your
airtime with and the payment details in section 4 are for paying
your future telephone bills. Please make sure that you COMPLETE
SECTION 5 IN ORDER TO PAY FOR THE ACTUAL PHONE* (and accessories
you may also wish to purchase). These details are totally
separate to how you choose to pay your service provider for calls
& line rental.
* Except if registering airtime only, without a provided handset.
3. The one off CONNECTION FEE of £35.25 and your subsequent
line
rental is paid to us at the time of connection. (Does not apply
to
FREE Phones/SIM only bundles - where you pay an all inclusive fee
as advertised)
4. The MINIMUM CONNECTION TIME to the network is 12 months,
with a
ONE month notice period. Therefore the MINIMUM TERM is 12
months (11+1 months).
5. YOU HAVE A 14 DAY CANCELLATION PERIOD, which starts from
the date
of connection. You may return the phone and any accessories that
you have purchased for a full refund, as long as you keep the
original packaging and return it in good condition.
6. INSURANCE: Unless you opt in for one of the various
Insurance
Cover Protection packages, you will accept that you will be
liable
for the extra cost of purchasing a replacement phone. Please
ensure
that you are aware of the replacement cost (shown as the
"Offline"
price within our price lists) before proceeding.
PROOFS:
7. You will also need to send us some proofs, which you should
return
with this form. For example, a drivers licence and BT phone bill.
* These proofs can include; A copy of the entire sheet of your
drivers licence (registered at the address of connection) to show
your signature and any Bank Statement or Utility Bill as another
proof of address. If you do not have a drivers licence, we can
accept a photocopy from your passport or the front & back of
a
valid credit card (not debit card). Call if you are uncertain.
8. A proper contract will be sent to you which needs to be
signed to
make this connection complete. You must return EVERY sheet of
this
contract to us - and we will return your copy with the relevant
tariff/handset information added to it upon connection.
9. You can either post, fax or Email the form back to us as
detailed further down;-
NOTES:
o All our phones are brand new and come with the standard
mains
charger and the standard battery. We do not, and never have, sold
reconditioned or second hand goods.
o You will automatically receive any free accessories from us
under
any promotion in place. In addition, any special offers that
apply
to you will be given automatically.
o Please photocopy this form if registering multiple handsets.
o Once you have completed the form and returned it, we will
use the
supplied information to go ahead and register the handset.
It is important that you check every section is completed
correctly as leaving information out (such as your date of birth
or a post code) will prevent us carrying out the credit check.

Free "Phone Me" Personal Number:
We provide a totally free "0700" personal number
with every connection,
although you are under no obligation to use it. This number can
be routed
to your mobile number & any 3 other UK destination Numbers
with no cost to yourself
now or in the future. Callers are charged at a higher rate
(comparable to
mobile rate) regardless of where you are actually routing the
call, which
is how the personal number company makes money from proving a
service at
no cost. You pay nothing to receive the calls and continue to get
the
callers Caller ID information if available.
To change the destination of your personal number, you are
given a unique
number that is PIN code protected. You can then change your
personal
number to divert calls (free of charge to yourself) to any UK
landline or
mobile phone number yor personal greeting on hold music ect.
The other advantage of using a personal number is clear should
you wish to
change phone or network in the future. Although your mobile
number may
change, callers will still reach you on your personal number. If
you would
like to have a special number, we offer a choice at extra cost
(from £25
upwards) for Fax & international numbers. Please note that
when you make outgoing calls from your chosen
destination, people will receive the "correct" Caller
ID information and
not your personal number.

DEPOSITS:
You may be required to pay a deposit before you will be
accepted as a
customer. However, this money can often be held as a bond if you
pay
by credit card and will be used to pay for your future airtime
bills.
Deposits paid by cheque, or for international roaming will be
held
for 12 months in the same was as with other companies.
The decision on the deposit is completely out of our control
and we
don't make any additional money out of you paying one. If you are
rejected for any reason, we can advise on how to get a copy of
your
credit history, which will cost 1-2 pounds sent to an agency.
