the form. It
will answer most common questions and may prevent you completing
parts
unnecessarily!
This form MUST be completed before we can supply a phone. You
will also
need to provide 2 proofs.
If you want to add a phone to an existing account, or upgrade,
please see
further on for information.
Personal users should complete sections 1, 3, 4 and 5 in all
cases.
Business users should complete sections 2, 4 and 5, with just
contact
details in section 1 (see section explanation below).
Please print out the form separately for multiple phone
connections. If on
the same account, complete one form and then attach a cover note
detailing
the extra handsets you require and the required talk plans and
user names
for each one.

The Sections Explained:
Section 1 is to be completed in all cases, but depending on
your order -
some bits may be left empty. Business users only need to
include a contact name, address and phone number.
Section 2 is for business users to complete. We will need an
official
order from you by fax or post, which may need to be given to
Orange.
Section 3 is to request a talk plan/tariff and any additional
services.
Some services are at extra cost and are explained accordingly
within this section.
Section 4 is your method of payment to Orange for phone
connection, line
rental and call charges.
Section 5 is for your actual phone order. Here you give us the
required
handset/additional accessories wanted and your payment details.
Section 6 is for requesting any special delivery instructions -
such as
a specific date for delivery or another delivery address.

Connection Notes:
The minimum connection time to the Orange network is 12
months, consisting
of a 1 month notice period, required in writing.
You have a 14 day money back guarantee for all new connections,
however
this excludes upgrades.
You may be required to pay a deposit by cheque or credit card
(held for 12
months) before you can be connected.
We require 2 proofs (Proof of address & ID) for all new
connections, which
must be sent by fax or post. Acceptable proofs are given below.

Talkshare:
Talkshare gives the ability to have more than one person
sharing the same
Talk Plan (tariff). When completing the form, complete it for the
account
holder as normal, but include the second person in the "User
Name" box. In
Section 3, mark the chosen Talk Plan as "Talkshare We provide a totally free "0700" personal number
with every connection,
although you are under no obligation to use it. This number can
be routed
to your mobile number any 3 other UK destination Numbers with no
cost to yourself
now or in the future. Callers are charged at a higher rate
(comparable to
mobile rate) regardless of where you are actually routing the
call, which
is how the personal number company makes money from proving a
service at
no cost. You pay nothing to receive the calls and continue to get
the
callers Caller ID information if available.
To change the destination of your personal number, you are
given a unique
number that is PIN code protected. You can then change your
personal
number to divert calls (free of charge to yourself) to any UK
landline or
mobile phone number.
The other advantage of using a personal number is clear should
you wish to
change phone or network in the future. Although your mobile
number may
change, callers will still reach you on your personal number. If
you would
like to have a special number, we offer a choice at extra cost
(from £25
upwards). Please note that when you make outgoing calls from your
chosen
destination, people will receive the "correct" Caller
ID information and
not your personal number.

Returning the Form/s to us:
Faxing back the form with your proofs is by far the quickest
way of placing
an order. Please note that we will generally only call you to
advise if
there are any problems with your order, not just to confirm
receipt. If you
specifically need us to acknowledge receipt, please state it
clearly
somewhere on the form when sending it.

Adding another phone to an existing account;
If you wish to add another phone to an existing account,
things are
very simple indeed.
Call Orange free on 180 using your handset and ask for them to
put a
note on the account stating that "C J M Communications will
be adding a
phone to the account". Now just complete the form including
your
account number, name, address and a user name for the new person.
If
Talkshare, state this in Section 3.
As of March 11th 1999, we will still require proofs or an
official order on headed paper.

The quick way to upgrading your Orange phone;
If you are upgrading your phone, you do not need to complete
this
form in full. Simply include your name, address and complete the
other
requested information shown in "For Upgrades Only".
We will then call Orange for authorisation and send you a hard
copy
contract for completion. Once you return this we can proceed to
get
the handset and a new SIM card to you. Please allow 5 working
days.
