Unfortunately many companies do not perform disaster recovery tests often enough. We recommend you test your recovery plan every six months. It is very tempting to assume that because a test worked one time, it will always work.
Things change in a production environment, and it's possible to forget about the recovery plan. If you use a backup machine and add disks to production, did you remember to do the same for the backup machine.
Tapes and tape drives do fail from time to time, and even if you monitor your backup logs it does not guarantee you will be able to restore from those tapes. When was the last time you tried to restore something from tape?
When was the last time you added a file system for database files, and was it added to the backup script ?
How long does it take to recover your system, and can your business afford to be without it's database for longer than you planned. As your database grows, so does the time it takes to restore.
These are basic issues but ones that are often forgotten. We can review your backup & recovery procedures, and if necessary help develop and test new ones.
